Insurance

We accept most insurance carriers.


Make sure we participate with your insurance plan. Please check with your plan about to find out if we are a contracted provider. You can do this by calling the member services phone number on the back of your insurance card. We will ask you for your card at EVERY visit even if coverage has not changed.

Billing


If we are not a provider for your insurance, if you are not insured or we do not file with your insurance company, you may be responsible for the entire charge at the time of service.
